Due to the ongoing Covid19 virus threat and trying to keep everyone safe, we have developed online forms that can be submitted to the Supervisor of Assessments Office electronically. Although the forms will still be available online to be printed off, we felt it was more convenient for the property owners to be able to fill the forms out and send them in one step.
Since the senior freeze form no longer needs to be notarized beginning in the year 2020, we have added the ability to sign the form by way of an electronic signature. This will help keep, not only our staff, but the senior citizens safe from the unnecessary trips out in the public. All exemption forms include an upload file button to attach proof of income, disability and age. If you would rather send the proof of income, disability, or age through the postal mail, please send it and a copy of the form that was submitted to: Peoria County Supervisor of Assessments, 324 Main Street, Room 301, Peoria, IL 61602.
